The growing geriatric population in the United States has created an increasing need for palliative medicine services across the range of medical and surgical specialties. Yet, palliative medicine lacks the resources to carry such a workload itself. Geriatric Palliative Care addresses this need by encouraging individual specialties to ""own"" the management of elderly with the same vigor as they ""own"" other key management competencies within their specialty. This clinically focused and highly practical handbook, which compliments the more comprehensive text Geriatric Palliative Care by Sean

Patients with advanced cancer may develop a number of clinical complications related to tumor progression or a variety of aggressive treatments. The majority of these patients are elderly, often with multiple co-morbidities that require appropriate assessment and management. In the palliative stage of their disease, patients undergo a progressive transition from active acute care to community-based hospice care. This transition requires modification in the diagnostic tests, monitoring procedures and pharmacological treatments to adjust them to the palliative and short-term nature of the care.

This practical manual presents the main drugs and protocols currently used in the psychopharmacological treatment of psychiatric disorders in cancer and palliative care settings and explores the principal issues involved in such treatment. Significant clinical challenges encountered in the psychopharmacological management of various psychiatric conditions are discussed, covering aspects such as side-effects and drug-drug interactions. Attention is also paid to the emerging theme of adjuvant use of psychotropic drugs for the treatment of symptoms or syndromes not primarily related to psychiatric disorders (e.g. pain, hot flashes). In addition, practical suggestions are provided for dealing with special populations, including children, the elderly, and people affected by severe mental illness. The book is designed to be easy to read and to reference: information is clearly displayed in concise tables and boxes, accompanied by further detail within the text and clinical vignettes. The authors include some of the most renowned clinicians working in the field of psycho-oncology. .

One of the most challenging roles of the psycho-oncologist is to help guide terminally-ill patients through the physical, psychological, and spiritual aspects of the dying process. Patients with cancer, AIDS, and other life-threatening illnesses are at increased risk for the development of major psychiatric complications, and have an enormous burden of both physical and psychological symptoms. Concepts of adequate palliative care must be expanded beyond the current focus on pain and physical symptom control to include the psychiatric, psychosocial, existential, and spiritual aspects of care. Hospice is the premiere end of life program in the United States, but its requirement that patients forgo disease-directed therapies and that they have a prognosis of 6 months or less means that it serves less than half of dying patients and often for very short periods of time. Palliative care offers careful attention to pain and symptom management, added support for patients and families, and assistance with difficult medical decision making alongside any and all desired medical treatments, but it does not include a comprehensive system of care as is provided by hospice. The many unfounded myths and fears that surround working with people at the end of their lives are dispelled in this thoughtful book, and the authors provide both practical and emotional support for those involved in caring for dying patients. The book covers everything from the philosophy behind the practice to the diverse roles in the multi-disciplinary team, as well as current challenges and opportunities facing end of life care, with an emphasis on how therapy and arts therapies can be integrated effectively. It also looks at palliative care models that address all aspects of a patient's wellbeing and conveys the latest research and challenges in the field, bridging the gap between theory and the realities of working with patients on a daily basis. Using material from artists and therapists working in the field, the varying demands of inpatient, outpatient and home care are described, as well as the processes of bereavement, how to handle the practicalities of loss and the help that therapists can give at this stage. How artists and therapists with experience of working at the end of life can be involved in other areas of care such as supporting older people in care homes and those living with dementia is also discussed. The book also introduces the important area of health promotion, and how there is a growing responsibility to change people's attitudes towards death, dying and bereavement. Clear, practical examples are given as to how this might be achieved.

Prognostication of acute neurologic disease is a major task for neurohospitalists and neurointensivists. The family conference raises the delicate matter of how to decide that outcome is indefinitely poor and how to define disability. This monograph is the first book on how to communicate prognosis in acute neurologic conditions. Communicating Prognosis includes useful data on prognosis in all major neurologic conditions and provides practical advice on how to effectively lead such a conference and how to resolve potential conflicts.
